COVID-19 Information

If you or your client are experiencing any symptoms listed below, please call our COVID-19 HOTLINE at (929)-388-0756 and report the symptoms to our Coordinators. They will relay the information to our Director of Patient Services’ (DPS), who will reach out to the patients or caregivers with the appropriate follow up measures.

Symptoms that may indicate COVID-19:

• Shortness of Breath
• Soreness
• Runny Nose
• Sore Throat
• Diarrhea
